for your initial visit to our practice are listed below. Click on the form, fill in the required information to print, save, scan or email it to info@dazzledentalcare.com.
If you prefer to email it, the data will be an (.xml ) file and a confirmation email will be sent once we receive it.
Also, you can print the form, fill in the data, scan the document as a (*.pdf) file and email it to info@dazzledentalcare.com
You can also print the blank form, fill in the required information and bring them to your next appointment.

Do i need to bring anything else to my appointment?Please bring the following: 1. Government issue ID2. Insurance Card or Information
